Job summary

Northeast Georgia Health System in Gainesville, GA is seeking a Case Management Social Worker to provide hospital-based medical social work services. You will assess patients across ages, collaborate with the care team, and develop discharge plans addressing social, financial, and environmental needs.

Responsibilities include crisis intervention, advocacy, and coordinating placements and community resources to support safe, timely transitions of care.

Qualifications

• MSW from an accredited School of Social Work; licensure preferred.
• Minimum 1 year hospital or health-care related social work experience.

Responsibilities

• Use family systems theory to assess social, psychological, cultural, environmental, and financial factors affecting care and discharge.
• Identify abuse or domestic violence and report appropriately.
• Assess for psychiatric or substance use disorders and arrange referrals.
• Apply SBIRT techniques to identify risky alcohol or drug use.
• Facilitate discharge planning through team collaboration and community resources.
• Coordinate placements (NH/SNF/AL/LTAC) and handle post-acute plans.
• Document updates in insurance, demographics, level of care, and ensure discharge documentation.
