Job summary

Northeast Georgia Health System is seeking a Case Management Support Specialist to assist Case Managers and Social Workers with discharge planning, continuum placement, and data gathering. You will work with physicians, payors, and families to coordinate post-acute services, transportation, and home health needs.

You will monitor patient care, promote quality outcomes, and ensure timely referrals, documentation, and follow-up across neonatal to geriatric populations, including weekend and remote

Qualifications

• High School Diploma or GED required.
• Two (2) years of healthcare experience required.
• Georgia LPN license or active Georgia license preferred.

Responsibilities

• Support Case Management/Social Worker with discharge planning and data collection.
• Coordinate post-acute placements and arrange discharge services (Hospice, DME, Home Health, transportation).
• Communicate with nursing homes and payors; document activities in the patient record.
• Provide patient care coordination and follow-up post-discharge.
