Job summary

Northeast Georgia Health System is seeking a Case Management Social Worker to deliver hospital-based medical social work across patient age groups, assess complex needs, and collaborate with the care team to plan safe discharges.

You will coordinate resources, advocate for patients and families, and ensure timely interventions and referrals on discharge, while maintaining alignment with safety guidelines and NASW ethics.

Qualifications

• Masters Degree in Social Work from an accredited School of Social Work; licensure preferred.
• Minimum of 1 year hospital experience preferred in a health care setting.
• CCM or ACM certification preferred.

Responsibilities

• Uses family systems theory to assess social, psychological, cultural, environmental and financial factors affecting care.
• Coordinate discharge planning and placement with the care team.
• Advocate for patient rights and ensure Freedom of Choice and Medicare's Important Message.
• Utilize SBIRT techniques for identifying substance use and facilitate appropriate referrals.
• Participate in rounds and communicate with the team to ensure timely discharge planning.
