Job summary

Northeast Georgia Health System is seeking a Case Management Social Worker to provide hospital-based medical social work services. You will assess patients’ social, psychological, and financial needs, collaborate with the care team, and develop discharge plans that meet clinical goals.

You will work across age groups, participate in rounds, coordinate with community resources, assist with indigent medications, and advocate for patient choices while maintaining NASW standards.

Qualifications

• MSW from an accredited School of Social Work; licensure preferred.
• Minimum one year hospital experience preferred.
• CCM or ACM certification preferred.

Responsibilities

• Assess patients’ social, psychological, cultural, environmental, and financial needs and discharge plans.
• Coordinate with care team to expedite discharge and arrange appropriate placements.
• Use SBIRT techniques for alcohol and other drug risk assessment.
• Provide patient advocacy and ensure Freedom of Choice and Medicare information is communicated.
• Attend multidisciplinary rounds and communicate needs clearly to the team.
• Assist with indigent medications and community resources for discharge placement.
• Provide therapeutic support and facilitate family/caregiver counseling as needed.
• Respond to referrals within 24 hours and maintain thorough documentation.
