Job summary

BayCare Health System in Dunedin, FL is seeking a Case Management Assistant to support patient access and transition planning. This PRN position runs 8:00 AM – 4:30 PM with occasional weekend shifts and no on-call duties.

Primary duties include coordinating services, distributing regulatory documents, and educating patients about discharge appeals. Requires 1 year in a medical-related field and a high school diploma; Associate's degree preferred.

Qualifications

• Required: High School or Equivalent.
• Preferred: Associate's degree.

Responsibilities

• Assist with facilitating patient and family access to agencies, transportation, and services within and beyond acute care.
• Distribute regulatory documents to identified patients.
• Educate patients about the appeal process related to discharge.
• Support seamless transition of patients from inpatient to community-based services.
• Maintain frequent contact with patients, families, hospital staff, and payor representatives.

The Case Management Assistant Responsibilities

• Assists with completing tasks necessary to facilitate patient and family access to agencies, transportation, and services within and beyond acute care
• Responsible for assisting with distributing regulatory documents to identified patients
• Educating patients about the appeal process related to their discharge
• Performing multiple activities that support a seamless transition of patients from the inpatient hospital setting, using current knowledge of hospital processes and community service
• Requires frequent contact with patients and patient representatives including but not limited to hospital staff, community resource agencies and payor representatives
