Job summary

BayCare Health System in the Tampa Bay area is seeking a Hospital Care Coordinator - RN Case Management to support discharge planning, care coordination and patient advocacy across a 16-hour weekly schedule at Wesley Chapel Hospital.

This role requires an RN license, 2 years acute care experience, and preferred ACM/CCM credentials, with additional education options. The position offers weekend coverage and a collaborative clinical environment.

Qualifications

• RN license required.
• ACM (Case Management) preferred.
• CCM (Case Manager) preferred.
• Associates of Nursing required; Bachelor of Nursing preferred.
• 2 years in Acute Care required.
• 2 years as Registered Nurse required.

Responsibilities

• Provides linkage, monitoring, planning and advocacy for our population to achieve and maintain maximum functioning.
• Possesses a clear understanding of the available resources to promote quality cost effective outcomes.
• Effectively maintains and manages assigned caseload, contingent on needs, strengths, abilities, barriers, and preferences of the individual served.
• Coordinates discharge planning efforts through assessment, referral, and interdisciplinary planning.
• Ability to adjust to changing priorities in a fast-paced environment.

Position details:

• Location: Wesley Chapel Hospital

• Status: Part time, 16 hours per week.

• Schedule: Saturday & Sunday - 8:00 AM - 4:30 PM

• Weekend Requirement: Every

Certifications and Licensures:

• Required RN (Registered Nurse)

Education:

• Required Associates of Nursing or

Experience:
