Job summary

AA Healthcare Management is seeking a Hospital Liaison to cultivate relationships with hospital case managers, social workers, and discharge planners. You will explain skilled nursing services to patients and families and support seamless transitions of care.

You will coordinate referral reviews, track clinical and financial eligibility, and ensure timely documentation while driving referral growth across the territory.

Qualifications

• Experience as Hospital Liaison or in a similar skilled nursing admissions/healthcare business development role.
• Knowledge of discharge planning, skilled nursing services, and Medicare/Medicaid/managed care payer requirements.
• Strong communication, relationship-building, organizational, and follow-through skills.

Responsibilities

• Develop and maintain relationships with hospital case managers, social workers, discharge planners, and other referral sources.
• Meet with patients and families to explain skilled nursing and rehabilitation services and assist with the transition of care.
• Coordinate referral reviews with Central Intake and facility teams to determine clinical and financial eligibility.
• Follow referrals through admission, communicate updates, and help resolve admission barriers.
• Maintain accurate, timely documentation in ExaCare.
• Support referral growth and meet established admission goals.
