Eastern Healthcare Group is seeking a Hospital Liaison to connect hospitals, physicians, case managers, and our skilled nursing facilities. You will develop referral relationships, facilitate admissions, and ensure smooth transitions from acute care to post-acute and long-term care services.

The role requires strong communication, relationship-building, and presentation skills, with at least 2 years in healthcare-related roles.

Qualifications

• Bachelor's degree in healthcare-related field preferred.
• Minimum 2 years of experience in healthcare sales, business development, admissions, or hospital liaison roles.
• Experience with skilled nursing, rehabilitation, long-term care, home health, hospice, or hospital systems preferred.
• Strong understanding of discharge planning and post-acute care services.

Responsibilities

• Develop and maintain relationships with hospital case managers, discharge planners, social workers, physicians, and healthcare professionals.
• Educate referral sources about the company's services, programs, and admissions process.
• Conduct regular hospital visits to identify referral opportunities and support discharge planning.
• Review referrals and collaborate with clinical/admissions teams for placement.
• Coordinate smooth transitions from hospitals to skilled nursing facilities.
• Maintain referral tracking and provide regular activity reports.
• Participate in networking events and community outreach.
• Stay informed on Medicare/Medicaid guidelines affecting post-acute care.
