Job summary

Center4Hospice seeks an experienced Visit Nurse to provide compassionate, patient-centered home hospice care in Mishawaka, IN. You will deliver direct and indirect nursing services per the interdisciplinary plan of care, document care at the point of care, and collaborate with the case manager and care team to support comfort and quality of life.

Qualifications

• Current Indiana RN license required.
• Michigan RN license a plus.
• Knowledge of hospice philosophy and regulations.
• Ability to work independently with an interdisciplinary team.
• Strong documentation and communication skills.

Responsibilities

• Provide direct and indirect patient care as outlined in the care plan.
• Document patient status and care interventions in the clinical record.
• Report to case manager and team with timely, clear updates.
• Attend meetings and participate in ongoing professional development.
• Maintain confidentiality and adhere to protocols and regulations.
• Support family education and self-care teaching.
