American RENOLIT Corporation is seeking a compassionate RN Case Manager to provide hospice care in patients' homes in Mishawaka, IN. You will coordinate plans of care, assess needs, and support families with clinical guidance and education.

Work independently in the field, partnering with an interdisciplinary team to ensure comfort, dignity and personalized end-of-life care. Maintain timely documentation and communicate clearly with patients, families and physicians.

Competenze

• Current Indiana RN license
• Michigan RN license is a plus
• Strong assessment, communication and critical-thinking skills
• Ability to work independently while collaborating with an interdisciplinary team
• Knowledge of hospice philosophy and applicable regulations is preferred
• Ability to meet background screening, drug screening and TB testing requirements

Mansioni

• Assess physical, psychosocial, environmental, safety and spiritual needs
• Develop and manage individualized plans of care based on patient and family goals
• Assess and manage pain, symptoms and comfort needs
• Provide skilled, compassionate nursing care in the patient’s residence
• Educate and support patients, families and caregivers as conditions change
• Explain medications and hospice services and what to expect in end-of-life care
• Coordinate care and communicate changes with physicians and the interdisciplinary team
• Participate in interdisciplinary group meetings to ensure coordinated care
• Supervise LPNs and hospice/home health aides per requirements
• Complete admissions and maintain accurate clinical documentation
• Serve as resource and preceptor for team members and students
