Center4Hospice is seeking a Registered Nurse Case Manager to provide compassionate hospice care in patients' homes. The role is in Mishawaka, IN, full-time, Monday–Friday, day shift, with on-call rotation and strong autonomy in the field.

You will assess needs, develop care plans, and coordinate with an interdisciplinary team to support patients and families through the end-of-life journey.

Qualificações

• Indiana RN license is required; Michigan RN license is a plus.
• Strong clinical assessment, communication and critical-thinking skills.
• Ability to work independently while collaborating effectively with an interdisciplinary team.
• Knowledge of hospice philosophy and applicable coverage requirements is preferred.
• Background screening, TB testing and employment eligibility requirements.

Responsabilidades

• Assess patients' physical, psychosocial, environmental, safety and spiritual needs.
• Develop and manage individualized plans of care with patient/family goals.
• Assess and manage pain, symptoms and comfort needs.
• Provide skilled nursing care in the patient’s residence.
• Educate and support patients, families and caregivers during changing conditions.
• Coordinate care and communicate changes with physicians and the hospice team.
• Participate in interdisciplinary meetings to ensure coordinated care.
• Supervise LPNs and aides per requirements.
• Complete admissions and maintain timely documentation.
