Job summary

Center4Hospice in Mishawaka, IN is seeking an experienced Registered Nurse Case Manager to support patients in their homes. This full-time, day-shift role offers autonomy and enables you to build meaningful relationships with patients and families while coordinating care with an interdisciplinary hospice team.

You’ll assess patient needs, develop individualized care plans, manage symptoms, educate families, and accompany patients through the hospice journey with compassionate, expert nursing

Qualifications

• Current Indiana RN license is required.
• Michigan RN license is a plus for cross-state coverage.
• Strong clinical assessment, communication and critical-thinking skills are essential.

Responsibilities

• Assess each patient’s physical, psychosocial, environmental, safety and spiritual needs.
• Develop and manage individualized plans of care based on patient and family goals.
• Assess and manage pain, symptoms and comfort needs.
• Provide skilled, compassionate nursing care in the patient’s residence.
• Educate and support patients, families and caregivers as conditions change.
• Coordinate care with physicians and the interdisciplinary team and communicate changes.
• Participate in interdisciplinary team meetings to ensure coordinated care.
• Supervise LPNs and hospice/home health aides per requirements.
• Complete admissions and maintain timely clinical documentation.

Education

Current Indiana RN license

Michigan RN license is a plus
