Job summary

Queen City Hospice is seeking a Hospice Transitional Care Navigator to drive upstream referrals, educate hospital and physician teams, and coordinate clinical assessments for hospice enrollment. This role emphasizes relationship-building, strategic outreach, and cross-functional collaboration in a non-remote market.

Qualifications

• Bachelor’s degree in marketing, business administration, or related field preferred.
• RN license in state of practice preferred.
• Minimum of two years relevant experience in clinical-sales or business development.
• Ability to make independent decisions and handle relationships tactfully.
• Valid driver's license and insurance required.

Responsibilities

• Build upstream-referral relationships with physicians, hospitals, and providers.
• Serve as clinical consultant and educator to hospital and care teams.
• Review clinical documentation to identify hospice-appropriate patients and enrollment potential.
• Educate care teams on hospice appropriateness and discharge education.
• Collaborate with field sales to schedule consultative meetings with external teams.
• Build care team workflows for information capture, education planning, and meetings.
