Job summary

Atlantic Visiting Nurse is seeking an experienced Hospice Referral Specialist RN to join our Care Access Center. You will act as the clinical bridge between hospitals, physicians, patients and families, assessing referrals, determining level of care, coordinating admissions, and ensuring timely, compassionate services for safe transitions from hospital to home.

Qualifications

• Active RN license in good standing
• Hospice or home care experience preferred
• Strong assessment and care coordination skills
• Proficient with Epic documentation

Responsibilities

• Conduct comprehensive clinical assessments to determine patient eligibility for appropriate level of care.
• Review medical documentation to ensure referrals meet admission criteria and regulatory requirements.
• Meet with patients and families to educate them on Hospice services, benefits, and care philosophy.
• Collaborate with physicians, case managers, discharge planners, and interdisciplinary teams to coordinate seamless transitions of care.
• Complete timely referrals and documentation within Epic.
• Coordinate durable medical equipment and other services needed prior to discharge.
