Job summary

JourneyCare Hospice in the assigned Illinois market seeks a Hospice Transitional Care Navigator to expand upstream hospice referrals from physicians and hospitals. This clinical-sales role focuses on education and building durable referral relationships within the DuPage County/West Suburbs area.

The ideal candidate is a self-driven, hunter-minded professional with strong communication, strategic thinking, and problem-solving skills.

Qualifications

• Bachelor’s degree in marketing, business administration, or related fields preferred, or equivalent experience.
• RN license in the practicing state preferred.
• Minimum of two years relevant experience.
• Ability to make independent decisions when needed.
• Ability to tactfully interact with patients, families, and providers.
• Knowledge of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance required.

Responsibilities

• Build upstream-referral relationships with physicians and referral sources.
• Serve as hospice clinical consultant and educator to hospital and physician care teams.
• Review hospital records to identify decline patterns and admission eligibility.
• Educate care teams on hospice appropriateness and discharge education.
• Identify attending and consulting providers for patient assessment and education.
• Collaborate with field sales to schedule consultative meetings with external teams.
• Maintain care team workflows for patient information and documentation.
• Identify trends and partnership opportunities for earlier hospice engagement.
• Foster trusted external relationships to improve patient outcomes.
