Job summary

Addus HomeCare in Mason, OH seeks a Hospice Transitional Care Navigator to expand upstream hospice referrals by partnering with physicians, hospitals and providers. You will assess clinical information, educate teams on hospice appropriateness, and drive patient-centered care across the care continuum.

The role requires clinical sales experience, strong communication, and a hunter-sales mindset to build trusted external relationships while upholding privacy and ethical guidelines.

Qualifications

• Bachelor’s degree or equivalent experience in marketing, business administration, or related field.
• Licensed registered nurse in the practice state preferred.
• Minimum of two years relevant experience in clinical sales or business development.
• Ability to make independent decisions and build referral relationships.

Responsibilities

• Build upstream-referral relationships and transition hospice referrals from physicians and other sources.
• Serve as hospice clinical consultant and educator with hospital and physician care teams.
• Review hospital records, engage with patients and providers to build clinical profiles.
• Identify trends and partnership opportunities for earlier hospice engagement.
• Collaborate with field sales to schedule consultative meetings with external care teams.
• Communicate clearly with hospital leadership and care coordination teams.
