Job summary

Queen City Hospice is seeking a Hospice Transitional Care Navigator to expand upstream referrals and educate physicians, hospitals and providers about hospice care.

The role focuses on building relationships, identifying suitable patients and ensuring timely transitions from acute care to hospice, while collaborating with hospital care teams and field sales to deliver seamless end-of-life care. This is not virtual and requires market territory presence.

Qualifications

• Bachelor’s degree preferred in marketing, business administration, or related fields.
• Licensed RN preferred.
• Minimum of two years relevant experience.
• Must possess the ability to make independent decisions when circumstances warrant.
• Must be able to deal tactfully with patients, family members, visitors, and staff.
• Must have valid driver’s license and proof of insurance.

Responsibilities

• A clinical-sales transitional care navigator/TCN who will build upstream-referral relationships and transition hospice referrals from physicians and other referral sources.
• Serve as hospice clinical consultant and educator, acting as extensions of hospital and physician care team.
• Review hospital documentation/EMR/HCHB, engage with patients and providers to build clinical profiles and admissions assessments.
• Consult with hospital care teams by sharing clinical profiles and educating on hospice appropriateness.
• Identify attending and consulting providers for patient assessment, eligibility and discharge education.
• Collaborate with field sales to schedule and conduct consultative meetings with external care teams.
• Initiate and maintain care team workflows for patient information/clinical assessment and documentation, education planning, and meetings.
• Identify trends, needs, and partnership opportunities for earlier hospice engagement.
• Foster trusted relationships with external care teams to improve patient outcomes.
• Communicate clearly with hospital leadership and care coordination teams.
• Organize, track and prioritize high-value patient reviews.
• Meet productivity requirements: timely medical record review, hospital/provider consultation and education.
• Promote collaboration and uphold policies.
• Represent the organization in hospitals and community interactions.

Education
