Job summary
Elara Caring is seeking a Transitional Liaison Registered Nurse to identify hospice-eligible patients, guide them through referrals and admissions, and coordinate comprehensive care transitions. You will assess physical, emotional, social, and spiritual needs and collaborate with care teams to align with patient goals.
Applicants must hold a current RN license and have at least 3 years of clinical experience, preferably in Home Health or Hospice.
Qualifications
- Must hold a current State RN license.
- Graduate of an accredited nursing program (Certificate, Diploma, ADN or BSN).
- Minimum 3 years of clinical care experience.
- Experience in Home Health or Hospice is preferred.
Responsibilities
- Identify patients who may qualify for hospice care.
- Follow hospice referral, admission and documentation processes.
- Assess patients' physical, emotional, social, and spiritual needs.
- Coordinate patient transitions and services with care teams.
- Meet with patients and caregivers to discuss goals and complete initial assessments.
- Explain hospice services so patients and families can make informed decisions.
- Educate staff and referral partners on hospice eligibility and transition processes.
