Elara Caring is seeking a Transitional Liaison Registered Nurse to lead hospice referrals, assess patient needs, and coordinate transitions across care teams. You will meet patients and families to discuss goals, explain services, and ensure smooth documentation and eligibility processes.

Requirements include an active RN license and 3+ years of clinical care experience; home health or hospice background is preferred. Benefits include comprehensive coverage, 401(k), and tuition reimbursement.

Competenze

• Active Registered Nurse license in the state.
• Graduate of accredited nursing program.
• 3+ years’ clinical care experience.
• Experience in Home Health or Hospice preferred.

Mansioni

• Identify patients who may qualify for hospice care.
• Follow the hospice referral, admission, and documentation process.
• Assess patients' physical, emotional, social, and spiritual needs.
• Work with care teams to coordinate patient transitions and services.
• Meet with patients and caregivers to discuss care goals and complete initial assessments.
• Explain hospice services so patients and families can make informed decisions.
• Educate staff and referral partners on hospice eligibility and the transition process.
