Elara Caring in the United States is seeking a Transitional Liaison Registered Nurse to support hospice transitions, identify patients who may qualify, and guide families through care options.

You will collaborate with care teams, complete initial assessments, educate staff and partners, and ensure compassionate, patient-centered care. Salary ranges and comprehensive benefits are offered.

Qualificações

• Current State license as a Registered Nurse.
• Graduate of an accredited nursing program (Certificate, Diploma, Associate or Baccalaureate).
• 3+ years’ experience in a clinical care setting.
• Experience in Home Health or Hospice preferred.

Job Description

As a Transitional Liaison Registered Nurse, you’ll contribute to our success in the following ways:

• Assess patients' physical, emotional, social, and spiritual needs.
• Work with care teams to coordinate patient transitions and services.
• Meet with patients and caregivers to discuss care goals and complete initial assessments.
• Explain hospice services so patients and families can make informed decisions.
• Educate staff and referral partners on hospice eligibility and the transition process.

What is Required?

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 3+ years’ experience in a clinical care setting
• Experience within Home Health or Hospice is preferred

The base salary for this position is $100K-$110K and is based on the company’s good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity.
