Job summary

Elara Caring is seeking a Transitional Liaison Registered Nurse to lead hospice referrals, assess patient needs, and coordinate transitions across care teams. You will meet patients and families to discuss goals, explain services, and ensure smooth documentation and eligibility processes.

What is Required?

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 3+ years’ experience in a clinical care setting.
• Experience within Home Health or Hospice is preferred.
