Job summary

Signature Eugene Hospice in Eugene, OR is seeking a compassionate Transitional and Bridge Care Coordinator RN to support seamless patient transitions from Home Health into hospice care. This role collaborates with our home health to coordinate referrals, facilitate admissions, and ensure timely, compassionate care.

The ideal candidate holds an Oregon RN license, has 1–2+ years of nursing experience, and excels in communication and coordination across teams to deliver exceptional patient outcomes.

Qualifications

• Current Oregon RN license.
• Graduate of an accredited nursing program.
• 1–2+ years of nursing experience required.

Responsibilities

• Coordinate hospice referrals and admissions from hospitals, SNFs, physician offices, and other partners.
• Educate referral sources on hospice services and patient eligibility.
• Collaborate with clinical and administrative teams to ensure exceptional patient care and customer service.
• Participate in community outreach, networking, and business development activities.
• Maintain accurate documentation and timely follow-up on referrals.
• Support agency growth by identifying opportunities to serve patients and partners.
