Enhabit Home Health & Hospice is seeking a dedicated professional to represent the branch in external transition care activities and educate patients, families, and health partners about our services.

You will develop patient-centered transition plans by applying clinical guidelines, protocols, and metrics to promote quality and efficiency, and act as a public awareness ambassador for our care offerings.

Qualifications

• Graduate of an approved school of clinical education (essential).
• Licensed in the state where they currently practice (essential).
• At least one year of experience in home care (essential).
• Knowledge of federal, state, and local hospice regulation (essential).
• Basic technology skills, including operation of a mobile device (essential).

Responsabilités

• Represent the branch in external transition care activities and educate patients and families about Enhabit Home Health & Hospice services.
• Develop patient-centered transition plans by integrating clinical guidelines, protocols, and metrics to promote quality and efficiency.
• Serve as a public awareness representative for provided services.

Formation

Graduate of an approved school of clinical education

Licensed in the state of practice

At least 1 year of home care experience

Knowledge of federal, state, and local hospice regulations

Basic technology skills (mobile device operation)
