Job summary

VIA Health Partners is a community-based, not-for-profit hospice and palliative care provider in North Carolina. We are seeking an experienced RN to deliver patient and caregiver care, conduct assessments, and develop care plans in collaboration with the IDG team.

The role emphasizes education, infection control, and interdisciplinary teamwork, with CHPN preferred and an active compact RN license. Strong communication and computer skills are required to support quality patient care.

Qualifications

• Diploma or Associate degree in nursing required; Bachelor's degree preferred.
• Active RN license in a compact multistate license (NLC) in home state; ability to practice in other compact states.
• Current CPR certification; CHPN preferred.
• Minimum 1 year experience in inpatient units (IPU) and 2 years for other nursing roles.
• Experience caring for patients with life-limiting illnesses; hospice/palliative care background preferred.
• Computer proficiency required.

Responsibilities

• Coordinate and deliver patient/caregiver care.
• Conduct nursing assessments and update the plan of care.
• Educate patients and caregivers; facilitate independence where possible.
• Communicate effectively with IDG, patients, caregivers, and health professionals.
• Maintain accurate clinical documentation and uphold infection control protocols.
