Job summary

VIA Health Partners is seeking a Utilization Review Nurse for Palliative Homecare in Shelby, NC. The role involves evaluating patient needs, coordinating with physicians, and educating families about palliative care plans.

Qualifications

• A Diploma or Associate Degree in Nursing is required
• A Bachelor’s degree is preferred

Responsibilities

• Delivery of patient care by evaluating patient/family needs, symptoms, and condition
• Liaises with primary care physician and palliative care providers regarding orders for the care plan
• Provides education and counseling to patient/family on palliative care and plan of care
• Coordinates community resources for patient needs with the social worker
• Ensures clinical documentation is accurate, complete, and timely per guidelines
• Maintains licensure, CPR certification, and CEU requirements
• Participates in interdisciplinary group conferences and referral processes

Education

Diploma or Associate Degree in Nursing
