Apploi is seeking a Hospice Chaplain to deliver and coordinate spiritual care for patients and families. You will assess needs, develop care plans, and provide direct spiritual support while collaborating with community chaplains and the interdisciplinary team.

The role emphasizes church/community outreach, documentation, and participation in care conferences, with a focus on interfaith sensitivity and ongoing professional development.

Qualifications

• Bachelor's degree in religious studies or related field required.
• Master's degree in theology, divinity or pastoral studies; 72 semester hours documented.
• Certification preferred; ordination or endorsement letter often required.
• Ongoing education and CPE experience encouraged.
• Hospice care experience preferred.
• Comfort with diverse lifestyles, cultures, beliefs, and values.
• Ability to network with community clergy and congregations.
• Knowledge of hospice philosophy of care.
• Strong verbal and written communication and organization skills.
• Offer contingent upon references and background checks.
• CPR certification maintained; valid driver with insured car.

Responsibilities

• Assure spiritual assessment of patients and families; deliver timely spiritual services.
• Develop individualized care plans by participating in comprehensive spiritual assessment.
• Provide direct spiritual care to patients and families.
• Liaise with community chaplains and spiritual counselors.
• Maintain records of spiritual care utilization for QA and policy review.
• Document direct services and ongoing communication with chaplains.
• Attend IDG meetings as part of the interdisciplinary group.
• Consult, educate, and support the interdisciplinary group on spiritual care.
• Recruit community chaplains and volunteers to meet needs; educate congregations.
• Provide funeral or memorial services as requested.
• Plan periodic memorial services for staff, volunteers, and clergy.
• Assist in supervising spiritual care volunteers with patients/families.
