Job summary

Community Healthcare of Texas is seeking a Spiritual Care Coordinator to provide spiritual and emotional support to patients, families, and staff within hospice services. The role involves connecting families with spiritual resources, participating in care planning, and offering bereavement support in collaboration with the Bereavement Coordinator.

Qualifications include a Bachelor's in Religious Studies, with preference for a Master's in progress (CPE units), and experience in grief counseling

Qualifications

• Bachelor's degree in Religious Studies required; substitution: non-religious degree plus 10+ years direct spiritual care experience accepted.
• Master's degree with Clinical Pastoral Education units preferred.
• Grief counseling experience preferred.
• Hospice experience strongly preferred.
• Proficient in using a computer.
• Excellent listening skills.
• Reliable transportation and a valid driver's license.

Responsibilities

• Assess patient/family with spiritual care needs; participate in the plan of care.
• Provide spiritual and emotional support according to the plan of care.
• Participate in on-call rotation and various schedules/duties as assigned or needed.
• Document visits/phone calls according to Policy.
• Participate in Interdisciplinary Team meetings.
• Provide aftercare and bereavement support in cooperation with the Bereavement Coordinator.
• Provide spiritual and emotional support for staff.
