Job summary

Community Healthcare of Texas seeks a Spiritual Care Coordinator to deliver spiritual and emotional support to patients, families, and staff within our hospice and palliative care program. You will help connect families with spiritual resources and provide ongoing bereavement care across care teams.

The role emphasizes collaboration with interdisciplinary teams, on-call duties, and ensuring compassionate spiritual guidance in end-of-life care.

Qualifications

• Bachelor’s degree in Religious Studies required.
• Substitution: non-religious bachelor's + 10+ years of direct spiritual care experience available.
• Master’s degree with Clinical Pastoral Education units preferred.
• Grief counseling experience preferred.
• Hospice experience preferred.
• Proficient computer skills.
• Excellent listening skills.
• Reliable transportation and a valid driver’s license required.

Responsibilities

• Assess patient/family in spiritual care needs and participate in care planning.
• Provide spiritual and emotional support per the plan of care.
• Participate in on-call rotation and varied schedules as assigned.
• Document visits and calls according to policy.
• Participate in Interdisciplinary Team meetings.
• Provide aftercare and bereavement support with the Bereavement Coordinator.
• Provide spiritual and emotional support for staff.

Community Healthcare of Texas has provided hospice and palliative care services since 1996. Our mission is to provide compassionate care for individuals living with serious illnesses while supporting their loved ones who care for them.

Community Healthcare of Texas is currently recruiting for a Spiritual Care Coordinator.

POSITION SUMMARY

The Spiritual Care Coordinator provides spiritual and emotional support for patients, families, and staff; assists in connecting families with avenues of spiritual support and provides ongoing bereavement care.

RESPONSIBILITIES

• Assess patient/family with spiritual care needs; participate in the plan of care
• Provide spiritual and emotional support according to the plan of care
• Participate in on-call rotation and various schedules/duties as assigned or needed
• Document visits/phone calls according to Policy
• Participate in Interdisciplinary Team meetings
• Provide aftercare and bereavement support in cooperation with the Bereavement Coordinator
• Provide spiritual and emotional support for staff

POSITION QUALIFICATIONS

• Bachelor’s degree in Religious Studies required.
  • (Will consider substituting a non-religious studies bachelor’s degree plus 10+ years of direct spiritual care experience for a Religious Studies degree)
• Strongly prefer a Master’s degree with or working on Clinical Pastoral Education units
• Grief counseling experience preferred
• Hospice experience strongly preferred
• Must be proficient in using a computer
• Must have excellent listening skills
• Must have reliable transportation and a valid driver’s license
