Job summary

Heart to Heart Hospice is seeking a compassionate Spiritual Care Coordinator to assess spiritual needs, coordinate with local clergy when needed, and develop individualized plans of care for patients and families.

You will provide prayer, worship, and ritual services, offer counseling and crisis support, assist with advance directives and funeral planning, and work with the care team to address ethical concerns while ensuring timely documentation.

Qualifications

• Must be ordained or endorsed in a religious role or have certified pastoral care training.
• 1+ year experience in healthcare or end-of-life care.
• Comfort working in interfaith settings.

Responsibilities

• Assess hospice patients and families to identify spiritual needs and create an individualized plan of care.
• Conduct services of prayer, worship and ritual for patients and families.
• Provide counseling, crisis intervention, and assistance with advance directives and bereavement support.
• Collaborate with staff to resolve spiritual/pastoral/ethical issues and ensure timely documentation.
• Be available for bereavement or counseling needs and participate in visits as needed.
