Job summary

VNA Hospice in New Hampshire seeks a Spiritual Bereavement Coordinator to provide compassionate bereavement and spiritual care to patients, families, staff, and volunteers. You will assess needs, coordinate services, and document care plans within an interdisciplinary team.

The role requires a relevant graduate degree (Thanatology or related field), with at least two years of spiritual counseling experience and two CPE units.

Qualifications

• Graduate degree in a related field is required.
• Minimum of two years of spiritual counseling and grief/bereavement coordination experience.
• Two units of CPE required; ordination or credentialing preferred.

Responsibilities

• Assesses, plans, implements, and documents spiritual and bereavement care for patients and families.
• Provides spiritual and bereavement care as part of the hospice team with documentation for each contact.
• Identifies spiritual/religious needs of patients and bereaved caregivers.
• Coordinates and supervises spiritual care in home and inpatient settings with local clergy.
• Interviews, screens, trains, and supervises volunteers for spiritual care programs.
• Ensures policies include spiritual/bereavement discussions and develops new ones as needed.
• Offers workshops on bereavement and spirituality to volunteers at least twice yearly.
• Maintains caregiver communication through assessments and contacts for at least one year.
• Plans and oversees Grief Recovery Support Groups and community events.
