Qualifications

• Master's degree in Social Work.
• Hospice experience of 1+ year.
• Licensed Social Worker (LSW) and/or Licensed Clinical Social Worker (LCSW) credentials.

Responsibilities

• Provide psychosocial assessments of patients and families to identify needs.
• Develop bereavement plans addressing family/caregiver needs.
• Document assessments and interventions in the patient’s EMR promptly.
• Collaborate with interdisciplinary teams to support care planning.
