Job summary

Carle Health is seeking a Home Services Social Worker to assess psychosocial needs of patients and families within a hospice-focused, interdisciplinary team. The role requires Hospice experience of 1+ year and offers a sign-on bonus.

Master’s Degree in Social Work with LSW/LCSW licensure is required; driver’s license and auto insurance proof are also needed. This position emphasizes team collaboration and comprehensive care planning within Carle Health.

Qualifications

• Master’s Degree in Social Work required.
• Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) licensure per IDFPR.
• Hospice experience of 1+ year is required.
• Ability to work as part of an interdisciplinary team.

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental needs.
• Develop a bereavement plan of care addressing family/caregiver needs.
• Educate patient and family members, respecting faith and culture while addressing barriers to learning.
• Document psychosocial assessments, financial assessments, and MSW interventions in the EMR.
• Support patient decisions and collaborate with the IDT to plan care.
