Qualifications

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental needs.
• Develop a bereavement plan of care for family members and caregivers.
• Educate patients and families, addressing learning needs and cultural beliefs.
• Document assessments and MSW interventions in the EMR in a timely manner.
• Implement a social work plan to maximize benefits and coping for patients and families.
