Elite Hospice, part of LHC Group, is seeking a Social Worker to provide medical social services under physician direction and with an Interdisciplinary Group to support patients and families. You will help interpret social and emotional factors related to health and help develop coping strategies.

Qualifications

• Master's Degree from a social work program accredited by the CSWE.
• 1+ years of social work experience in a health care setting.
• Current Driver's License, vehicle insurance, and access to reliable transportation or public transit.

Responsabilités

• Assist the patient, family, physician, and care team in understanding social, emotional, and environmental factors affecting health.
• Contribute to development of a comprehensive Plan of Care for patients daily.
• Instruct care team on community resources available to assist patients as needed.
• Serve as Bereavement Coordinator and establish a care plan addressing bereavement needs.
