Job summary

Elite Hospice, part of the UnitedHealth family, seeks a Social Worker - Per Diem to provide medical social services under physician direction and an Interdisciplinary Group. You’ll help patients and families cope with social and emotional factors affecting health.

You will assess needs, plan daily care, and guide the team on community resources. This role includes bereavement coordination and flexible per diem hours.

Qualifications

• Bachelor of Social Work OR Master of Social Work.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.
• Licensed Social Worker in the state of practice.
• 1+ years of social work experience in a health care setting.
• Bereavement Coordination experience.
• Experience with establishing a plan of care for bereavement needs.

Responsibilities

• Assist patient and family to understand social factors related to health.
• Contribute to development of a comprehensive Plan of Care.
• Plan of Care for patients on a daily basis.
• Instruct team on community resources available for bereavement needs.
• Possible Bereavement Coordination and supervision of bereavement services.
