Qualifications

• Current driver's license and reliable transportation.
• CO: Licensed Social Worker.

Responsibilities

• Understand social and emotional factors affecting patient health status and coping.
• Contribute to a comprehensive Plan of Care with health care team daily.
• Instruct team on community resources to assist patients.
