Job summary

UnitedHealth Group in Bryant, AR is seeking a Social Worker - Hospice Per Diem to provide medical social services under physician direction and the Interdisciplinary Group, helping patients manage significant social and emotional factors related to health status.

Responsibilities include assessing patients and families, contributing to the Plan of Care, guiding community resources, and coordinating bereavement services as needed.

Qualifications

• Master's Degree from an accredited School of Social Work.
• 1+ year of social work experience in a health care setting.
• Current Driver's License, vehicle insurance, and access to dependable transportation.

Responsibilities

• Assist patient, family, physician and health care team to understand social factors affecting health status.
• Contribute to development of a comprehensive Plan of Care for patients on a daily basis.
• Instruct health care team on community resources available to assist patients as needed.
• Able to function as Bereavement Coordinator and plan bereavement services reflecting patient family needs.
