Baptist Trinity Hospice is seeking a Social Worker - Hospice Per Diem to provide medical social services under the direction of a physician and the interdisciplinary group. You will help patients and families understand social and emotional factors related to health status and contribute to a comprehensive Plan of Care.

The role includes bereavement coordination, development of care plans, and guiding the team on community resources to support patients and families.

Qualifications

• Master's Degree from a school of Social Work accredited by the Council of Social Work
• 1+ years of social work experience in a health care setting
• Current Driver's License and access to dependable transportation

Responsabilités

• Assists patient and family to understand social and emotional factors related to health status
• Contributes to an integrated Plan of Care with the care team on a daily basis
• Instructs team members on community resources to assist patients as needed
• Can function as Bereavement Coordinator and develop a bereavement care plan

Formation

Master's Degree in Social Work

Licensed Social Worker (preferred)

CPR certification
