The role focuses on understanding significant social and emotional factors impacting health and developing coping strategies for patients and families.

Key duties include supporting patients and families, contributing to a Plan of Care, coordinating bereavement services when needed, and guiding the care team to connect with community

Qualifications

• Master's Degree from a school of Social Work accredited by the Council of Social Work

Responsibilities

• Assists patient, family, physician and care team to understand emotional and social factors affecting health status
• Contributes to a comprehensive Plan of Care for patients daily
• Instructs care team on community resources to support patients
• Bereavement coordination when needed and plans of care for bereavement needs
