Job summary

UnitedHealth Group is seeking a part-time Social Worker to provide medical social services under physician direction, contributing to an integrated Plan of Care and linking patients to community resources. The role emphasizes collaboration with a multi-disciplinary team and 20–25 hours per week.

Required qualifications include an MSW from a CSWE-accredited program, a valid driver’s license, and CPR certification (or within 90 days of hire).

Qualifications

• Master's Degree from a CSWE-accredited School of Social Work.
• Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
• CPR certification or ability to complete within 90 days of hire.
• Licensed Social Worker in the state of residence.

Responsibilities

• Assists the patient, significant others, physician and health care team staff to understand significant personal, emotional, environmental and social factors related to the patient's health status on an as needed basis.
• Contributes as a health care team member to the development of a comprehensive, integrated Plan of Care for patients on a daily basis.
• Instructs health care team members on community resources available to assist patients on a as needed basis.
