Job summary

AdvisaCare Home Health & Hospice is hiring a Medical Social Worker (MSW) to support patients and families in the Traverse City region. The role offers flexible part-time/per diem opportunities and a chance to make a meaningful impact in the local hospice community.

The MSW will conduct psychosocial assessments, develop care plans with the IDT, and provide crisis counseling as needed. A CSWE-accredited MSW and valid driver's license are required, with home health and hospice experience preferred.

Qualifications

• MSW degree from CSWE-accredited program.
• One year of Home Health Care experience preferred.
• Hospice experience preferred.
• Valid Driver’s License, car insurance and reliable transportation.

Responsibilities

• Performs psychosocial assessments and helps develop goal-directed care plans.
• Provides care under physician direction and participates in interdisciplinary team conferences.
• Documents visits, calls and referrals accurately in patient records.
• Conducts reassessments of patient/family needs and counseling as required.
• Provides crisis intervention and individual or family counseling when indicated.
• Acts as a care advocate for patient/family unit and coordinates with community resources.
• Provides ongoing bereavement support as needed.
• Adheres to Hospice policies and participates in team initiatives.
