Job summary

Elite Hospice, part of LHC Group, seeks a Part-time Social Worker to provide medical social services under a physician and interdisciplinary team. Responsibilities include assessing social and emotional factors affecting health, aiding in care planning, and guiding staff to community resources. Requires a MSW and active driver’s license.

Salary ranges from $66,400 to $99,600 annually for full-time equivalents; benefits and opportunities for growth are provided. Equal opportunity employer.

Qualifications

• Master’s Degree from an accredited School of Social Work.
• Current Driver’s License with reliable transportation.
• Licensed Social Worker in the state of residence.
• 1+ year of social work experience in healthcare.
• CPR certification or ability to obtain within 90 days.

Responsibilities

• Assist patients and families to understand social factors related to health status and coping.
• Collaborate with physician and interdisciplinary team to develop Plan of Care.
• Educate staff on community resources to support patients and families.
