AdvisaCare Home Health & Hospice is hiring a Medical Social Worker (MSW) for the Gaylord, MI area. This part-time/per diem role focuses on compassionate, one-on-one care and supporting patients and families through difficult moments.

The MSW will perform psychosocial assessments, participate in care planning, and coordinate resources with the hospice team, while maintaining documentation and patient advocacy. A CSWE-accredited MSW and a valid driver's license are required.

Formación

• CSWE-accredited MSW degree required.
• One year of home health care experience preferred.
• Hospice experience preferred.
• Valid driver’s license required.

Responsabilidades

• Performs psychosocial, emotional, and bereavement assessments and assists in planning goal-directed care.
• Documents patient/family visits and actions timely and accurately in the record.
• Coordinates with physicians and IDT to support psychosocial needs.
• Provides crisis counseling and ongoing support to patients and families.
• Maintains compliance with Hospice policies and community referrals.
