Job summary

AdvisaCare is seeking a compassionate Medical Social Worker (MSW) in Michigan to support patients and families facing life-limiting illness within a hospice setting. This role focuses on psychosocial assessment, planning, and counseling in a one-on-one home health environment.

Ideal candidates hold an MSW from CSWE, with preferred hospice or home health experience and a valid driver’s license. The position features flexible part-time/per diem scheduling and a supportive team.

Qualifications

• MSW degree from CSWE-accredited program.
• One year of Home Health Care experience preferred.
• Hospice experience preferred.
• Valid driver's license, car insurance and reliable transportation.

Responsibilities

• Performs psychosocial, emotional, spiritual and bereavement assessments and contributes to goal-directed care plans.
• Documents all patient/family visits, calls, and referrals in the chart.
• Conducts reassessments and provides counseling when indicated.
• Provides crisis intervention and individual or family counseling as needed.
• Acts as a care advocate for patients and families; maintains community referrals.
• Collaborates with Bereavement Team and adheres to hospice policies.
