AccentCare is seeking a full-time Social Worker/MSW to join our hospice team in King of Prussia, PA. You will collaborate with an interdisciplinary group to plan care, support patients and families, and participate in discharge planning within a compassionate hospice setting.

The role requires an MSW and state licensure, with at least two years of healthcare social work experience preferred. Grief counseling and discharge planning experience are a plus, and driving to multiple locations may be

Qualifications

• Master's Degree in social work from an accredited program.
• Must be licensed to practice in the state(s) of agency operation.
• At least two years of professional social work experience (post-graduate) in a healthcare setting.
• Grief counseling experience preferred.
• Discharge planning experience preferred.

Responsabilités

• Assist the core members of the pre-hospice/hospice team in understanding social, spiritual, and emotional factors related to health to establish a plan of care.
• Participate as a member of the interdisciplinary team and in the plan of care development and review.
• Assess social, spiritual, and emotional needs to estimate capacity and coping with terminal illness.
• Prepare the patient and family for end-of-life changes and advance care planning.
• Utilize resources to support the patient and family within illness limitations.
• Support the bereavement program.
• Provide discharge planning related to level-of-care changes.
• Provide supervision of interns and SW fellows as needed.
