Job summary

AccentCare is hiring a Social Worker (MSW) for hospice care in Largo, MD. This full-time role supports patients and families, collaborates with the interdisciplinary team, and guides discharge planning and care coordination across Montgomery County.

The ideal candidate holds a Master’s Degree in Social Work, an active license, and healthcare experience; Medicare/Medicaid knowledge is preferred. Grief counseling and family support are valued aspects of this position.

Qualifications

• Master’s Degree from an accredited school of Social Work.
• License/registration to practice in state(s) of operation.
• Healthcare setting experience preferred; grief counseling experience a plus.

Responsibilities

• Assist pre-hospice/hospice team with social, spiritual, and emotional factors in care plans.
• Collaborate with the interdisciplinary team to develop and review plans of care.
• Assess needs to support coping with terminal illness for patients and families.
• Educate families on advance directives and care planning.
• Utilize community resources to support patients and families.
• Support the bereavement program.
• Provide discharge planning related to level-of-care changes.
