Job summary

UnitedHealth Group is seeking a Social Worker to provide medical social services under the direction of a physician and the Interdisciplinary Group to address social and emotional factors affecting the patient's health status and coping mechanisms. As a member of the care team, you will help develop a comprehensive Plan of Care and educate the team about community resources.

The role requires MSW from CSWE-accredited program, a valid driver’s license, and CPR certification or ability to obtain

Qualifications

• Master’s Degree from CSWE-accredited school of Social Work.
• Current driver’s license with access to reliable transportation.
• CPR certification or ability to obtain within 90 days.
• Licensed Social Worker in the state of residence (preferred).

Responsibilities

• Assists the patient, significant others, physician and health care team staff to understand significant personal, emotional, environmental and social factors related to the patient's health status on an as needed basis
• Contributes as a health care team member to the development of a comprehensive, integrated Plan of Care for patients on a daily basis
• Instructs health care team members on community resources available to assist patients on a as needed basis
