Qualifications

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional and social needs.
• Develop a bereavement plan of care addressing family needs.
• Educate patients and families with sensitivity to beliefs and learning styles.
• Document assessments and MSW interventions in the EMR.
• Develop and adjust the social work plan in collaboration with the IDT.
