Job summary

Qualifications

• Master's degree in Social Work required.
• LSW or LCSW license required or pending licensure.
• Hospice experience of 1+ years is essential.
• Ability to work as part of a multidisciplinary team.

Responsibilities

• Provide psychosocial assessments of patients and families.
• Develop bereavement plan of care for family/caregivers.
• Educate patients and families about learning needs and cultural beliefs.
• Document assessments and interventions in the EMR.
• Collaborate with IDT to implement social work plan of care.
