Job summary

The Home Services Social Worker at Carle Health identifies psychosocial needs of patients and families through assessment and provides social work interventions, including resource support and short-term counseling, as part of an interdisciplinary team. Hospice experience of 1+ year is required, and a sign-on bonus is offered.

Educational requirements include a Master’s Degree in Social Work with licensure as an LSW or LCSW as applicable.

Qualifications

• Hospice experience of 1+ years.
• MSW required.
• Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) in the relevant jurisdiction.

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental needs.
• Develop a bereavement plan of care to address family member needs.
• Interpret and communicate patient/family cultural Traditions and spiritual beliefs.
• Educate patients and families to overcome barriers and meet learning needs.
• Document psychosocial assessments and interventions in the EMR.
• Collaborate with the interdisciplinary team to implement the social work plan and support patient decisions.
